Pretty sure that's HLMV, SFM has a different kind of shading.

Your image looks much better than the previous one, so good job. My only criticism is that you're not running HLMV with Very High quality textures (the Medic's emblem shouldn't be that pixelated).

Here's how the emblem should look like in very high texture settings:

You can try downloading my Pre-configured dxsupport.cfg to see if that fixes it (make sure to backup your dxsupport.cfg before, just in case).

It was made in HLMV. I just downloaded the config that was linked to me and that has fixed the pixelated medic badge problem.

Should I update this image later with the very high textures or is it fine as it is?

Also, why do you think it was made in SFM? Is there something that I could have done better?

The pose looks odd to me, it looks like something made specifically for the cosmetic. Nothing about the quality or anything, no worries.

Well, if the file I sent to you works like the picture I sent to you, then you can replace it if you wish, but it's otherwise fine as is. Though, high quality textures will look better for your future uploads, if you're planing on doing new pics.

Changing my mat_picmip setting to anything lower than 0 causes the gray cornea issue. I fixed that with the fix provided on a different page, I just hope that not many other things are broken the same way.

Only a few cosmetics/weapons (Chops Pipe and Quick-Fix are some examples) have the same problem, but no new cosmetics (ever since the Workshop integration) seems to have the same issue.

If you ever feel like needing to equip a weapon on an image, always make sure to use Stock weapons (that also means, no festive/botkiller attachments). So there won't be any issue with Quick-Fix since you wouldn't be using it anyway.
