The Ubersaw is an unlockable melee weapon for the Medic. It is a cleaver-like bloodied blade fixed into a further blood-stained Bonesaw grip. Its most prominent feature is a large syringe filled with team-colored, glowing liquid and the needle protruding out of the front of the weapon. The needle ends in a hollow point and is also splattered in blood.

Every successful hit against an enemy player with this weapon adds 25% to the player's ÜberCharge meter, at the cost of a 20% slower swing rate. ÜberCharge is gained on hit even if the victim is invulnerable or Cloaked, but not if the victim is disguised. Its critical hit animation is an upwards sawing motion, similar to the animation used by the Bonesaw. The Ubersaw does not build the ÜberCharge meter in Medieval Mode.

The vial on the Ubersaw's syringe serves as a visual representation of a Medic's ÜberCharge. It fills slowly as teammates are healed and enemies are attacked with the weapon. Should the ÜberCharge be deployed and the player switch to the Ubersaw, the liquid can be seen slowly draining from the syringe.

Taunting with the Ubersaw equipped will perform the Spinal Tap kill taunt. Taunting can be achieved by pressing Alt-Fire on the weapon. A successful kill with this taunt will give a Medic 100% ÜberCharge. However, killing a disguised Spy will only award 50% in total. The initial hit will grant 25% Übercharge and stun the enemy. Players whom are stunned can be freed by the attacking player being killed, stunned, taunt-stunned or air-blasted during the taunt. The following kill will grant 75% Übercharge and kill the enemy. It is possible to kill multiple enemies with this taunt. The attack and retraction do not both have to both be executed on the victim for the Übercharge to be received. Doing only the former will award 25% Übercharge and doing only the latter will award 75% Übercharge.

The Ubersaw is automatically given to any player who obtains 22 Medic achievements.
