"A Little Heart to Heart" is currently available as one of the main menu's startup themes. It is one of two songs featured in Meet the Medic, the other one being "MEDIC!". It is listed as track number thirteen and is one of 32 main menu startup themes. It is the tenth track to be added via a patch subsequent to the game's release.

Another theme, "Archimedes", contains elements from this track and has been added as main menu startup music, but has not been used in a Meet the Team video.

"A Little Heart to Heart" is reminiscent of a polka style, with a strong downbeat bass and with accompaniment playing on offbeats. The track demonstrates the ever-dwindling psyche of the Medic, and follows suit by its eventual devolvement into madness.

The song starts with a traditional accordion accompaniment, with clarinet eventually joining to play the main melody. The melody and accompaniment both stay in the key of F minor, though playing around with chromaticism before a brief pause. The track is brought back to life by strings, with its quicker, slightly staccato darkening the mood, and the choir joins the clarinet and accordion on the main theme. The strings act as a guide for tension, the tempo increasing exponentially as the clarinet melody from the beginning repeats. As the suspense hits its climax, the song quickly calms down, falling into a gritty, yet-resolved section which carries us to final musical statement of the track made by choir. Extra importance is placed on the choir by the melody dropping out, and the accompaniment droning a chord accompaniment. The final choir statement begins in F minor, but within the last two measures ends in F major.
