"A Little Heart to Heart" is currently available as one of the main menu's startup themes. It is one of two songs featured in Meet the Medic, the other one being "MEDIC!". It is listed as track number thirteen and is one of 32 main menu startup themes. It is the tenth track to be added via a patch subsequent to the game's release.
Another theme, "Archimedes", contains elements from this track and has been added as main menu startup music, but has not been used in a Meet the Team video.

The song starts with a traditional accordion rhythm section, accompanied by a melodic brass section. The usage of brass as the main melodic focus contrasts with its use as a rhythm instrument in other tracks, and helps to give the song its distinctive alpine feel. The song stays in the key of F minor, playing around with the different chord degrees before descending into a brief pause. The track is brought back to life by an arpeggiated string section, with its quicker, slightly staccato pacing darkening the mood dramatically. This darker tone is more in-line with the rest of the Team Fortress soundtrack, further supported by more rhythmic brass stabs. The strings act as a guide through the sea of instrumentation, the tempo increasing exponentially as the melody from the start of the song repeats. Almost immediately as the suspense hits its climax, the song calms down, falling into a gritty-yet-resolved groove. This interlude carries us to the final build, introducing us to a choir that finalizes the song; extra importance is placed on the choir by way of cutting the surrounding instruments out.
