The Super SMG, also known as the SMG 2, is a scrapped primary weapon for the Medic. It used the same model as SMG. The weapon was scrapped for unknown reasons and replaced with the Syringe Gun before the final release.
As the name suggests, Super SMG was used for a version of the Super Nailgun from Team Fortress Classic, but with the SMG model. This means that the weapon may have had a higher rate of fire and more bullet damage than the stock SMG.
