Item sets, depending on the set, may apply new attributes to the wearer when all the items in a set are equipped, altering the role of the class slightly. The Spy has several item sets, all of which are listed below.

- Primary – L'Etranger: +15% Cloak on hit; -20% damage penalty.
- Melee – Your Eternal Reward: Upon a successful backstab you rapidly disguise as your victim; Silent Killer: No attack noise from backstabs; the wearer cannot use the Disguise Kit.
- Headwear – Familiar Fez: A headwear item for the Spy.
