The Horrific Headsplitter is an all class hat released with the 2010 Scream Fortress update. It appears as a cleaver embedded in the class's skull with blood pouring from the wound. It can be purchased from the Mann Co. Store with no deadline. It can also be found via the random drop system or crafted using the fabricate headgear recipe.
