The Nine-Pipe Problem is a community-created miscellaneous item for the Medic. It is an old-fashioned smoking pipe and dark glasses that replaces the Medic's usual clear glasses.
The Nine-Pipe Problem was contributed to the game via the Steam Workshop, under the name "The Baskerville Case".

Trivia
- The item is a reference to Sir Arthur Conan Doyle's fictional detective Sherlock Holmes.
- The dark glasses and the pipe were first popularised by the Template:W of the the short stories.
- The phrase "three pipe problem" was introduced in the short story Template:W. Sherlock confesses that this was the amount of tobacco needed to ponder over and deduce the solution to a particular puzzle. A "nine pipe problem" therefore would be fiendishly difficult to solve.
- Additionally, the nine in the title may be in reference to the 9 individual classes of opposing team the Medic must consider.
