The Medic

The Operation Two Cities Badge is a cosmetic item for all classes. It is a large gray disk featuring Archimedes, complete with blood splatters on his chest and face, perched on a laurel with a five-pointed star, from which a shield adorned with the Medic's class emblem hangs on a chain.

This item is awarded to players when they complete their first mission of the Operation Two Cities Tour of Duty in Mann Up mode. It tracks a player's progress through the various Mann vs. Machine missions. The badge's level increases based on the number of Tours completed (with the total number of Tours being the item's level minus one).
